Reports from the OSHP are that on December 11, 2013 at approximately 12:43 pm, a 2004 Chevy Ventura minivan was being driven by the woman, north on State Route 586. She allegedly drove the vehicle off the right side of the roadway and lost control. The minivan struck a few small trees. Howard Street towed the vehicle.
